The adrenal gland produces hormones that affects growth, development and stress, and also helps to regulate kidney function. There are two parts of the adrenal glands, the adrenal cortex and the adrenal medulla. The adrenal cortex produces mineralocorticoids, which regulate salt and water balance within the body, glucocorticoids (including cortisol) which have a wide number of roles within the body, and androgens, hormones with testosterone-like function.[2] The adrenal medulla produces epinephrine (adrenaline) and norepinephrine (noradrenaline).[2] Disorders of the adrenal gland may affect the production of one or more of these hormones. The adrenal glands are stress glands—they are a key part of the body’s response to physical and mental stress. When you’re in a stressful situation, your adrenal glands help your body adapt to the stressor. If a tiger is chasing you, your adrenal glands will produce cortisol, adrenaline, and other stress hormones. HUMAN ENDOCRINE SYSTEM :ADRENAL GLAND :They are a pair of yellowish, flat, pyramid like glands which lie over the upper end of the kidneys. They look like a cap on the kidney. Each gland has an outer yellow adrenal cortex and inner reddish brown adrenal medulla. Adrenal Medulla secretes 2 hormones called adrenaline or epinephrine and non adrenaline or non epinephrine. These are commonly called catecholamines. In conditions of physical and mental stress such as fall in blood sugar levels or when a person is injured or exposed to cold conditions or suffering with pain. It is also secreted during anger, fear or grief. It helps the body to face these conditions. It is therefore called Emergency hormone or Fight or Fight hormone or 3F-hormone (Fight, fright and flight). These hormones increase alertness, pupilary dilation, raising of hairs, sweating etc. the hormone increases blood supply to heart and skeletal muscles. It constricts arterioles and blood supply to skin and gastrointestinal tract. The bronchioles dilate. There is increased rate of breathing, oxygenation and heart beat to meet any emergency. Catecholamines stimulate the breakdown of glycogen resulting in increased concentration of glucose in blood. They also stimulate the breakdown of lipids and proteins. Adrenal cortex : can be divided into 3 layers called zona reticularis inner layer. Zona fasiculata middle layer. Zona glomerulosa outer layer. Adrenal cortex secretes many hormones called corticoids. Gluco corticoids : involved in carbohydrate metabolism. E.g. cortisol. Mineralo corticoids : corticoids which regulate the balance of water and electrolytes in our body. Eg: Aldosterone. Gluco corticoids stimulate gluconeogenesis, lipolysis and proteolysis and inhibit cellular uptake and utilization of amino acids. Cortisol is also involved in maintaining the cardiovascular system and kidney functions. It also produces anti inflammatory reactions and suppresses the immune response. Cortisol stimulates RBC production. Aldosterone acts at renal tubules and stimulates reabsorption of Na+ and water and excretion of K+ and〖Po〗_4^-. Thus aldosterone helps in the maintenance of electrolytes, body fluid volume, osmotic pressure and B.P. small amounts of androgenic steroids are also secreted by adrenal cortex which play a role in the growth of axial hair, public hair and facial hair during puberty.

The adrenal glands are small glands located on top of each kidney. They produce hormones that you can't live without, including sex hormones and cortisol. Cortisol helps you respond to stress and has many other important functions. With adrenal gland disorders, your glands make too much or not enough hormones. In Cushing's syndrome, there's too much cortisol, while with Addison's disease, there is too little. Some people are born unable to make enough cortisol. Causes of adrenal gland disorders include • Genetic mutations • Tumors including pheochromocytomas • Infections • A problem in another gland, such as the pituitary, which helps to regulate the adrenal gland • Certain medicines Treatment depends on which problem you have. Adrenal cancer is the presence of malignant adrenal tumors, and includes neuroblastoma, adrenocortical carcinoma and some adrenal pheochromocytomas. Most adrenal pheochromocytomas and all adrenocortical adenomas are benign tumors, which do not metastasize or invade nearby tissues, but may cause significant health problems by unbalancing hormones. Adrenocortical carcinoma (ACC) is a rare, highly aggressive cancer of adrenal cortical cells, which may occur in children or adults. ACCs may be "functional", producing steroid hormones and consequent endocrine dysfunction similar to that seen in many adrenocortical adenomas, but many are not. Due to their location deep in the retroperitoneum, most adrenocortical carcinomas are not diagnosed until they have grown quite large. They frequently invade large vessels, such as the renal vein and inferior vena cava, as well as metastasizing via the lymphatics and through the blood to the lungs and other organs. The most effective treatment is surgery, although this is not feasible for many patients, and the overall prognosis of the disease is poor. Chemotherapy, radiation therapy, and hormonal therapy may also be employed in the treatment of this disease.[citation needed]

The pituitary gland regulates adrenal gland hormone release via negative feedback. The adrenal glands release aldosterone, cortisol, and testosterone to help us deal with stress. Aldosterone increases kidney retention of salt and water and therefore can increase blood pressure. Cortisol mobilizes fat stores and increases blood glucose by stimulating lipolysis and gluconeogenesis. Testosterone prepares the mind and body for physical conflict. A number of disorders can affect the adrenal glands, including tumors. Tumors can be either benign or malignant. Benign tumors aren't cancer. Malignant ones are. Most adrenal gland tumors are benign. They usually do not cause symptoms and may not require treatment. Malignant adrenal gland cancers are uncommon. Types of tumors include • Adrenocortical carcinoma - cancer in the outer part of the gland • Neuroblastoma, a type of childhood cancer • Pheochromocytoma - a rare tumor that is usually benign Symptoms depend on the type of cancer you have. Adrenal glands tumours tend to be very small, especially the benign ones the malignant ones tend to be large, that is how we differentiate between benign and malignant tumours so typically a tumor more than 5 or 6 cms in size has to be considered malignant unless otherwise proven, so we need to excise the tumour. But smaller size tumours can be left alone and very often these benign adrenal tumours are recognized incidentally. So when you go for a scan for some other reason, that is unrelated to the Adrenal gland, you may find out on an ultrasound or a CT scan that there is a small tumor. So this is called as an incidnetalomas. So typically a benign adrenal tumours are asymptomatic, that is what you have to keep in mind, whatever symptom you may get may not be related to the adrenal gland tumour itself. You need to see if your adrenal gland tumour is giving you any trouble, you need to look at your blood pressure and if it is easy to control the blood pressure because blood pressure can be elevated even without adrenal gland tumour. Most people who have high blood pressure do not have adrenal gland tumour. But is your blood pressure difficult to control. If it this is difficult to control with multiple drugs then the tumor could be responsible for your blood pressure and we need to look at this carefully. Do you have features of Cushing’s syndrome? Cushing’s syndrome can be diagnosed easily by your endocrinologist by just looking at you. So if there is a good index of suspicion, we do a test called as the screening test. The usual screening test that we do is a 24 hour collection for cortisol or we do an overnight dexamethasone suppression test, by these tests we come to know whether you have cortisol excess. If you have cortisol excess, you will have all the symptoms of Cushing’s syndrome like rounding of face outing on weight in the belly, having stretch marks in your skin, thinning out of skin, weak bones fractures, all these can be features of adrenal tumors which are benign. But if any adrenal tumors are producing symptoms, they need to be removed. If any adrenal tumor is producing symptom unrelated to it, then probably removing it may to help it. So typically the link between the symptom and the adrenal tumour is a difficult one to make. It is best left to a specialist. It is best to visit an endocrinologist if you have a benign adrenal tumour that has been discovered.
